What are the options for follow-up care?
There are several ways of delivering follow-up care after discharge from hospital, depending on what’s needed. They include home hospitalization or services delivered by a home healthcare provider or private / agency nurse. Additional home help services may also be required.
What is ELSAN dom?
ELSAN has selected and approved a number of home healthcare providers to help coordinate, simplify and improve the ‘return home’ for patients, working with the hospital doctor and care team, local practitioner, agency nurses and pharmacy. These providers enable us to implement and ensure continuity of care in the home, throughout the ongoing care journey of each patient.

What kinds of patients is it for?
Patients hospitalized for the treatment of acute or chronic conditions: cancer (chemotherapy), obesity (nutrition, enterostomal therapy), heart or respiratory failure, diabetes (insulin pumps) and postoperative follow-up (enterostomal therapy, nutrition, healing). And patients dependent on others for assistance (home support).
How is ELSAN dom offered to patients? And how much does it cost?
This care package is prescribed by your practitioner and delivered by the healthcare team at your ELSAN facility.
You don’t have to pay anything — patient care, coordination and at-home follow-up services provided by a healthcare partner are paid for in France by the CPAM health insurance fund.

What kind of home care is included in the package?
ELSAN dom covers respiratory assistance, home chemotherapy, pain relief, artificial nutrition, insulin therapy, enterostomal therapy, post-operative healing and home support equipment.
Which home healthcare partners are affiliated with ELSAN dom?
ELSAN works in partnership with ELIVIE, ORKYN, HOMEPERF, Bastide Médical, FSK, AS Médical and Perfulor.
Each partner has signed up to ELSAN’s quality charter, which covers all aspects of the home healthcare services they provide.
Working with ELSAN, they’re investing in the provision of personalized assistance and the technologies needed to stay in touch with patients and empower them to play an active role in their care.
